Waiver Information

According to DMAS, on July 1, 2017 the Elderly or Disabled with Consumer Direction (EDCD wavier) and Technology Assisted waivers were combined into the new Commonwealth Coordinated Care Plus Waiver. This change offers the same services to beneficiaries of both waivers in one singular program.

There are six insurance companies who provide managed long-term services and support program coverage under the CCC Plus Wavier Program. Key Elements Care, LLC is currently credentialed with the six companies as follows:

What is the Commonwealth Coordinated Care Plus Waiver Waiver?

The CCC Plus Waiver is a Medicaid funded program for the elderly and persons of all ages with disabilities. Under waiver services individuals can choices to receive services in their homes. If the individual would like to hire their own staff to provide care for them. These services are called Consumer Directed Services.

Admission Process- Two Parts

Step 1: Contact your local Social Services to schedule an appointment with a Family Services Specialist to assess you needs for services. The Department. Of Social Services works in collaboration with the Virginia Health Department. The Uniform Assessment Instrument (UAI) is used to determine eligibility for waiver.

Step 2: Apply for Medicaid using any of the following methods

1. Online – CommonHelp (https://commonhelp.virginia.gov) – Apply for Assistance.

2. Or Call (855) 242-8282 (available in English and Spanish) to apply for long-term care benefits.

3. On paper – Bring a completed Medicaid application for Adult Medicaid and Appendix D to your Local Department of Social Services (DSS) and request a screening for Long Term Care.

Consumer-Directed Care

Consumer Directed Services- Consumer Directed Services are designed to let the individual and/family caregiver control the direction of services by hiring their own staff member(s) to assist in the home.

Key Elements Care, LLC assists individuals and/or family members in acquiring services through the CD waiver program. We serve as the liaison between you and Medicaid by facilitating paperwork and filing requests with the Virginia Department of Medical Assistance Services (DMAS). PCG Public Partnerships, LLC (PPL) is the DMAS fiscal agent, contractor who handles all payments related to reimbursement related to services provided by the attendant working with the individual.
